The HLM Healthcare Sector Action Team (HSAT) primarily consists of healthcare providers and administrators from hospital systems, clinics and health plans as well as nonprofit and academic partners. The HSAT was born out of the July 2015 Full Collaborative Meeting where the Prevention Institute presented their framework for Community Centered Health Homes (CCHH), and healthcare providers across Houston/Harris County explored the role they could play in addressing community health outside the four walls of a clinical setting.  Since then, the team has grown to now include over 32 organizations working to address childhood obesity by focusing on “upstream” social factors. 

Current outcome objectives of this team:

  • Develop consensus recommendations to advance standardized approaches and metrics for screening social determinants of health including  food insecurity across Harris County healthcare providers
  • Develop consensus recommendations to advance standardized approaches that addresses other social factors related to obesity across Harris County healthcare providers
  • Develop a guidebook relevant to local providers to advance adoption of CCHH/community-clinic linkage (CCL) policies and practices
